Are HR document templates free of charge?

Yes, all document templates available on this website are completely free. You can download them in PDF (ready to print) or Word (DOCX, editable) format. We regularly update the database, adding new patterns adapted to current labor law regulations.

Are the documents current for 2026?

Yes, the templates are updated on an ongoing basis in accordance with applicable labor law and tax regulations for 2026. This includes, among others: PIT-2 form version 9, PPK applications and personal questionnaires. You will find the date of the last update of each document next to its description.

Is a job candidate's personal questionnaire mandatory?

Yes, the personal questionnaire for the candidate and employee results directly from the Labor Code (Article 22¹ of the Labor Code) and the regulation on personal files. The employer is entitled to request the indicated data, and the documents must be stored in parts A and B of the personal file.

What is PIT-2 and when should an employee submit it?

PIT-2 (version 9) is a declaration authorizing the employer to reduce the monthly tax advance by a reducing amount (1/12 of the tax-free amount). The employee submits it before the first payment or after a change in circumstances (e.g. taking up additional work). The form is valid indefinitely until the declared data changes.

When must an employer issue a referral for medical examination?

Initial referral - before the candidate is allowed to work in a given position. Periodic examinations - periodically, according to the schedule specified by the occupational medicine doctor. Control examinations - when the employee was on sick leave for more than 30 days. The referral template can be found in the Employment (Onboarding) tab.

How often should the declaration of resignation from PPK be renewed?

The declaration of resignation from PPK is valid for 4 years. After this time, the employer is obliged to enroll the employee again (automatic enrollment). The employee may then submit a new declaration of resignation. It is worth keeping an eye on these deadlines - failure to re-register may result in financial penalties.

Does an application for annual leave have to be in writing?

The Labor Code does not require a written form, but the vast majority of employers use written or electronic applications for record-keeping purposes. A written application protects both the employee (confirmation of submission) and the employer (leave records). We provide the ready-made pattern in PDF and DOCX formats.

What is the difference between termination of a contract and termination by mutual consent?

The notice of termination is a unilateral declaration of the employee - the employer must accept it, the contract terminates after the expiry of the notice period (1-3 months depending on the length of service). The parties' agreement requires the consent of both parties, but allows for any termination date to be set - even immediate. Both templates can be found in the Contract Termination tab.
